Position Summary

The Family Resource Worker (FRW) provides, system navigation, and resource support to children, youth, and families within Neurodevelopmental Health

The FRW carries a caseload and supports both targeted and complex needs, with a focus on reducing barriers to accessing services and connecting families to appropriate community resources. A key responsibility is helping families access and secure funding for equipment and related needs, including coordinating applications, gathering required documentation, and liaising with clinicians and funding agencies.

The successful candidate will also serve as a resource to staff by providing guidance on community services and system navigation and will maintain in-depth knowledge of specialized and community-based resources to meet the unique needs of each child and family.

Main Responsibilities

Complete assessment of family needs, priorities, and available resources.
Provide short-term case management and system navigation support.
Support families in accessing healthcare services, particularly those without a primary care provider and/or who are new to Canada/Ontario.
Assist families in addressing community-based needs including housing, transportation, respite, and recreation.
Support families experiencing socioeconomic barriers using culturally safe and inclusive practices.
Support applications to government and community funding programs (e.g., equipment, supplies, financial supports).
Coach families to complete applications independently whenever appropriate.
Coordinate required documentation in collaboration with clinicians.
Liaise with vendors and funding agencies to support approval and payment processes.
Connect families to community resources including recreation programs, camps, respite services, and community health centres.
Develop and maintain up-to-date resource materials and packages for families.
Identify interim solutions and pursue special funding requests (e.g., transportation, emergency needs).
Liaise with NDH clinicians and Social Workers to confirm needs and align support.
Act as a community resource navigator and consultant for staff.
Maintain confidentiality in accordance with organizational policies and procedures related to privacy.
Because of the changing nature of the job and the job requirements, duties and responsibilities may be added or removed from time to time.

Qualifications

College diploma in Social Services, Developmental Service Worker, or related field (Essential)
Minimum three (3) years of recent experience supporting children and families (Essential)
Current CPR Certification (Preferred)
Experience in case management and system navigation (Essential)
Experience working with children/youth with neurodevelopmental needs (Essential)
Experience supporting families facing socioeconomic barriers and/or newcomers to Canada (Essential)
Strong knowledge of community resources and services for children and youth. (Essential)
Knowledge of funding programs and application processes (e.g., ACSD, SSAH, OAP, IFHP). (Essential)
Excellent communication, coaching, and problem-solving skills. (Essential)
Strong organizational and time management skills. (Essential)
Ability to work independently and collaboratively within an interdisciplinary team. (Essential)
Strong documentation and computer skills (Microsoft Office Suite). (Essential)
Valid driver’s license and access to a vehicle. (Essential)
Ability to travel for occasional community visits (Essential)
Vulnerable Sector Check (Level 3) (Essential)
Bilingualism (French/English) (Essential)

Please note that location may be subject to change at any time at the discretion of the employer. All OPSEU members shall be granted an interview upon passing the pre-screening.  Pre-screening might include testing.

Effective March 29, 2021, CHEO will conduct a more comprehensive pre-screening and evaluation process.  This process may include more varied and multidimensional testing such as a written test, practical exam, or other assessment. All candidates who meet the essential and preferred qualifications may be tested.

Bilingualism (French/English) will be validated by a language proficiency assessment conducted by a third party provider. Candidates will be required to meet the minimum linguistic profile prior to the date of hire.
