About the Opportunity

Reporting to the Health Services Manager (HSM), the Frailty Coordinator provides leadership and support in the development, implementation, evaluation, and ongoing advancement of Early Mobility Programs and other initiatives identified by the Nova Scotia Health (NSH) Frailty Network and zone leadership. The Frailty Coordinator works closely with zone leadership teams, as well as other NSH zone leaders, coordinators, and the Frailty and Eldercare Network, to contribute to the development of provincial programs, policies, and processes related to frailty. The role supports increased organizational awareness of frailty through strategies focused on identification, management, and prevention across community, long-term care, and acute care settings, while upholding the Mission, Vision, and Values of NSH. A key focus includes leading inpatient mobility teams throughout the zone, providing education to front-line staff of various disciplines on the use of the Clinical Frailty Scale (CFS), and collaborating with Clinical Nurse Leads, Unit HSMs, and interdisciplinary teams on the implementation and evaluation of frailty-related work including principles of Dignity of Risk. The Frailty Coordinator also provides clinical guidance to mobility staff, including education, orientation, and real-time problem solving as issues arise, and supports provincial, zonal, and site-level frailty initiatives in collaboration with interdisciplinary teams. The Frailty Coordinator is responsible for overseeing the coordination, training, implementation, and ongoing development of mobility program members, as well as providing education to frontline staff and clinicians on the care of hospitalized older adults. The Frailty Coordinator demonstrates competence in relevant clinical interventions, such as contributing to care plan development and supporting patient mobilization within the scope of their professional license or registration. The Frailty Coordinator acts as the point person for the frailty program and is responsible for day to day operations including: acting as a two-way conduit of communication between the HSM and service areas; troubleshooting to ensure consistent provision of services across the department; and ensuring the provision of safe and effective care.

About You

We would love to hear from you if you have the following:

Bachelor’s Degree in Occupational Therapy, Physiotherapy, Nursing, or other Regulated Health Profession required
Maintains current license/registration with the Nova Scotia College and maintains eligibility for membership in the Canadian Association
Minimum of three (3) years of relevant experience in health‑care service delivery, quality improvement, and program development, including program design, implementation, and evaluation.
Demonstrated excellence in verbal and written communication, with strong diplomacy and interpersonal effectiveness in complex clinical and organizational environments
Demonstrated leadership qualities with a thorough understanding of team leadership models and interprofessional collaboration.
Proven ability to assume a leadership role, providing direction, mentorship, and support to staff across multiple practice settings.
Demonstrates clinical expertise sufficient to support effective service delivery, clinical decision‑making, and evidence‑informed practice within the interdisciplinary team
Competencies in other languages an asset, French preferred

Compensation and Benefits

$38.43 - $50.99 Hourly
