Program Description:
The Guelph-Wellington HART Hub is an integrated and interdisciplinary primary care and social service program that provides continuous stage-matched care to individuals seeking treatment, recovery, and access to permanent housing. The services offered at the HART Hub include primary care and acute episodic healthcare, mental healthcare and treatment, addictions care and treatment, and housing services for adults and youth (age 16 and older) living in Guelph and Wellington County and who are members of our priority populations.
Reporting to the Supervisor, the HART Registered Practical Nurse
will provide support and education to individuals accessing the HART Hub services. They will use their unique personal and lived experiences to build positive relationships and connections with clients. They will work with individuals
along the substance use continuum and will act as an integral part of the team providing wrap around care for clients.
Knowledge, Skills and Abilities:
- Registered Practical Nurse, member of the Ontario College of Nurses
- Experience working with people along the substance use continuum
- Experience supporting clients with high levels of complexity related to their physical health, mental health, substance use and access to basic needs
- Excellent interpersonal and communication skills
- Strong ability to work with and build rapport with members of our priority populations
- Understanding of Indigenous Cultural Safety principles and trauma-informed care.
- Ability to create and maintain clear professional boundaries with people.
- High level of confidentiality.
Principal Job Duties:
- Works as part of an integrated care team
- Works at the top of scope of RPN practice
- Provides safe administration of medications and immunizations, venipuncture, disinfection/sterilization of equipment and environment, client education and assists practitioners as requested.
- Triage clients to prioritize urgent needs
- Ensures adequate supply of common medications and medical supplies
- Perform phlebotomy and collection of other specimens for testing
- Wound care
- Support crisis intervention, code response, and de-escalation
on site, as required
