The HST Outreach/LPN will report directly to the Balwin/Health Support Team (HST) Program Manager. The successful candidate will have an active license to practice in Alberta and be in good standing with their licensing body. It is preferred that the person have 2-3 years related experience working under a harm reduction model. The role will be responsible for providing daily assistance as needed in a trauma informed, harm reduction model which will include medication administration, wound care, enhancing quality of life, maintaining hygiene, managing symptoms, and assisting in activities of daily living. He or she will perform administrative duties for the HST including charting in ETO and FYI, maintaining referrals in the program, and helping the Program Manager prepare stats and reports.
Description of Duties and Responsibilities
•Outreach Nurse/LPN reports directly to the Program Manager
•Ensures case notes and charting are completed using the Efforts to Outcomes (ETO) data collection system.
•Co-ordinates care with Agency staff and external health care providers to ensure program participants have appropriate supports arranged.
•Takes and records measurements of blood pressure, temperature, heart rate, vital signs etc.
•Assists ailing patients in daily necessary activities such as eating, bathing etc.
•Administers injections, prescribed medications, enemas etc., cleans and dresses wounds and assists with other basic medical care tasks
•Provides emotional and psychological support when needed
•Responsible for giving daily medical and personal assistance as needed in a harm reduction model which will include medication administration, wound care and other medical assistance
•Provides occasional transportation and arranges warm hand offs of clients for medical, wellness and recovery appointments
•Participates in learning and training opportunities as identified on yearly performance review
•Updates and maintains information on referrals sources using the FYI and/or ETO database
•Completes medical referrals and assessments
•Interacts with HST clients in a non-judgmental way, using de-escalation and rapport building techniques
•Assists in Activities of Daily Living
•Co-ordinates with interdisciplinary team such as physicians, AHS staff, case managers, home care workers, etc.
•Provides necessary support in crises and emergencies
•Participates in team efforts to maintain a healthy physical environment for our clients
•Attends staff meetings as required
•Any other tasks assigned by the Program Manager
Position Qualifications
•Ability to undertake a variety of diverse care tasks and work in a multidisciplinary team
•Understanding of confidentiality obligations and nursing best practices
•Good knowledge of MS Office and data entry
•Excellent communication and interpersonal skills
•Compassionate and able to handle stress
•Successful completion of LPN license program is a must
•Demonstrates understanding of the addictions field
•Proven leadership skills
•Valid Driver’s License
•Own vehicle with $2 million liability insurance to transport clients as needed
•Experience working with people with complex life circumstances associated with substance-related disorders, mental health issues, mobility issues and homelessness
•Basic understanding of harm reduction principles based on dignity, hope and healing as individuals move forward towards improved health and well-being
•An open, accepting, and non-judgmental attitude with an ability to build rapport with clients and staff
•Current and clear Vulnerable Sector Police Information Check
•Understanding of the Truth and Reconciliation Commission of Canada (TRC) as well as experience working with Indigenous community members
•First Aid/CPR/AED Training
Hours of work: Shift work.
Permanent Part-time Sunday to Wednesday, 3:45pm-10:15pm, 6 hours.
Full-time 3 Month Contract Tuesday to Saturday 1:45pm-10:15pm, 8 hours.
