Title: Child and Youth Mentor
Position Type: Flexible Opportunities available for both Employees and Independent Contractors.
Location: GTA and surrounding areas
Salary: $22 to $25 per hour, depending on experience
Position Summary
The Child and Youth Mentor is responsible for delivering individualized support and mentorship to children, youth, and
adults diagnosed with Autism Spectrum Disorder (ASD), Developmental Disabilities, Mental Health Disorders, and/or Dual
Diagnoses. Acting as a positive role model, you will implement customized service agreements and foster trusting
relationships to promote the well-being and development of clients in staff-model residential homes and community
environments.
Key Responsibilities
Individualized Support and Crisis Intervention:
Develop and deliver individualized support plans tailored to the unique goals and needs of each client.
Provide one-on-one mentorship following a "Big Brother & Big Sister" model to guide youth development.
Implement crisis intervention and de-escalation strategies to manage acute behavioral challenges in a calm,
effective manner.
Foster positive, trusting relationships using positive reinforcement techniques to promote self-esteem.
Administer medication as prescribed and maintain accurate documentation.
Ensure safe transportation for clients to appointments and community activities.
Assist with housekeeping tasks, including meal preparation and cleaning, to maintain a healthy living environment.
Daily Living and Skill Development
Assist clients with activities of daily living (ADLs), including personal hygiene and feeding, as outlined in care plans.
Model and teach essential life skills aimed at enhancing safety, independence, and future employment pathways.
Encourage participation in activity programs to promote social interaction and community engagement.
Documentation and Administration:
Maintain accurate shift reports, incident reports, and medication logs to ensure compliance.
Track progress through a data-driven approach to celebrate client success and measurable outcomes.
Attend case conferences and contribute recommendations to multi-disciplinary care teams.
Adhere to all policies and procedures set forth by Rise & Reach Support Services.
Other Duties:
Performs duties as assigned.
Qualifications
Community College Diploma or University Degree in Social Services field.
Minimum of 1 to 2 years of experience supporting children, youth, or adults with complex needs.
Current Broad Sector Check required. Broad Sector Checks accepted within the last 6 months.
Willingness to obtain a current Child Welfare Record Check from local Children’s Aid Society.
Valid Standard First Aid and CPR Level C required.
Crisis training (CPI, UMAB, SMG, TCI).
Driver’s Abstract, Class G Driver’s License.
Access to an insured vehicle with $2 million liability insurance.
Up-to-date immunizations and TB test.
