The North Lambton Community Health Centre is a community governed health centre serving identified priority populations in a defined catchment area. The Centre operates multiple sites in Ontario: Forest, Kettle & Stony Point, Watford, and Sarnia. Our interdisciplinary team provides primary health care, health promotion and community development services.
Registered Dietitian - Diabetes Education
18 Month Contract 37.5 hours per week
The Registered Dietitian will work with the and Diabetes Education program to provide evidence-based disease self-management and education to individuals living with type 2 diabetes, and pre-diabetes. The main site of work will be the North Lambton Community Health Centre.
Qualifications
- Undergraduate Degree in Foods and Nutrition from a recognized university.
- Possess current registration with the College of Dietitians of Ontario (temporary licenses will also be considered).
- CDE certification, an asset but not required.
- Demonstrated skills in facilitating groups, team skills, and effective communication.
- Demonstrated knowledge of community-based health care and resources and the determinants of health.
- Experience in program planning, implementation, and evaluation.
- Proficiency in French is considered an asset
Flexible hours of work are required. The incumbent may be required to travel to all sites and outreach locations throughout Lambton County.
The rate of pay for this temporary fulltime position is $36.62 - $43.49 per hour (on a six-step grid) plus 10% in lieu of benefits. The rate will be based on education and experience. The Healthcare of Ontario Pension Plan enrollment is available.
