This primary care position involves outpatient chronic disease management in a multi-disciplinary team setting. The position will begin February 1, 2026 and will continue for 12 - 18 months.
*Position Summary:*
As a member of the multi-disciplinary team, the incumbent will work with patients who have been referred for, but not limited to;
diabetes, heart health,weight management and GI conditions in a variety of settings (individual, group and families). Resource, program development and implementation will occur as needed.
*Qualifications/Skills:*
* Bachelor of Science in Nutrition accredited by the College of Dietitians of Alberta (CDA).
* Registered with (or eligible to be registered with) the CDA.
* Certified Diabetes Educator would be considered an asset.
* Two years’ experience in chronic disease management in a primary care setting will be considered an asset.
* Ability to work effectively in a multi-disciplinary team environment.
* Excellent written, verbal, and interpersonal communication skills.
* Strong organizational skills.
* Strong analytical, assessment, and decision-making skills.
