Home & Community Care Program Manager (In-Training)
Position Summary:
This position will transition from Manager In-Training to the full working level of Manager, Home & Community Care through a planned succession process related to a retirement anticipated in December 2027. The successful candidate will work alongside the current Home & Community Care Manager and receive approximately one year of training, mentorship, and support to ensure a successful transition into the role. During the training period, the Manager In-Training will be supervised by the current Home & Community Care Manager.
The Home & Community Care Program Manager participates as a member of the management team providing second level support, advice, and program direction and coordination to the STC First Nation members.
The Home Care Manager plays a key leadership role in supporting the health and well-being of community members by overseeing the delivery of culturally safe, respectful, and holistic home and community care services.
The HCC Program Manager provides clinical supervision; program support to the member First Nations; collection, analyzation, interpretation and evaluation of program data as well as completion of reports to funding agencies and relevant working committees/partnerships within STC, as well as regionally and federally.
The Home Care Manager is responsible for the overall leadership, coordination, and delivery of home and community care services. This includes ensuring high-quality client care, advocacy, and maintaining compliance with all relevant health regulations and cultural protocols. The manager works collaboratively with health professionals, families, and community stakeholders to ensure services are responsive to the physical, emotional, and cultural needs of clients.
The HCC Program Manager provides program management; supervision and program guidance to:
- STC Community Dietician
- Health Promotion Coordinator/Community Dietician
- Maternal Child Health Coordinator
- Know Your Status Coordinator and
- Community Health Outreach Worker

Responsibilities:
Manage the Home & Community Care Program, the ADI program, the CPNP program, the Know Your Status program, the Maternal Child Health Program by: developing, coordinating and implementing policies, objectives and standards, providing strategic planning, developing program performance measurement strategies; providing leadership to the team
1. Supervision and clinical direction
- Provide leadership, direction; program delivery advice; clinical supervision; orientation, program audits and evaluation to the nurses who provide HCC client services and provide supervision to Unregulated Care Providers in communities (includes: on site in person; phone and email).
- Respond to clinical adverse events and reportable incidents in collaboration with the health care team members in the STC member First Nations and STC.
- Ensure HCC policies, goals and philosophy are developed, understood, and adhered to according to Best Practices.
- Ensure care is delivered in a respectful, culturally safe, and client-centered manner.
- Research and develop policies/forms accordingly for Best Practices and Standards of Care.
- Develop and support patient safety, risk management and quality improvement strategies.
- Manage the usage and provide guidance on the use and calibration of specialized equipment.
- Review and monitor health outcomes of HCC services such as; client advocacy, case management, chronic disease management, nursing, assisted living, personal care, home support, in-home respite and palliative care to ensure best practice and standards are being met.
- Conflict resolution and time management expertise.
- Ensure nurses have the competencies required for Specialty Practice Nursing procedures as outlined by the professional governing association.
- Ensure Medical Directives and Clinical Protocols for RN/LPN’s are adhered to.
- Facilitate and arrange a preceptor for practicum students upon request.
- Ensure that privacy and confidentiality is maintained.
2. Human Resources
- Provide consultation to First Nations Leadership and STC member First Nations on service development and provision issues related to recruitment and retention of staff, staff education, professional development, licensing and liability issues.
- Ensure a comprehensive orientation is provided.
- Manage and support human resources: establish goals, assess work performance, coordinate and monitor the work of staff, provide clinical performance evaluations, motivating staff to improve performance, monitor and assess program delivery, identify and recommend training needs and professional development programs, approve travel and expenses. (In member First Nations and STC).
- Develop training plans and education to maintain best practices and qualifications, provide professional support, clinical guidance and on the job training.
- Ensure delivery of continuing education for competency certification and promoting capacity building within the STC member First Nations.
- To attend continuing education sessions that will enhance professional skills.
3. Program Development and delivery
- Identify program concerns and work with the appropriate personnel to identify action plans to resolve the concern.
- Provide professional assistance in the development and delivery of the HCC program though needs assessments, plans and delivery, training and capital plans.
- Monitor program performance by monitoring budget reports, statistical reports, performance reviews/appraisals and client feedback.
- Maintain stats, complete monthly, quarterly and annual statistical reports, do data analysis and evaluate program outcomes.
- Complete monthly reports of work completed throughout the month to Governance House.
- Participate in the development of reporting systems for health indicators, utilization and service loads.
- Accreditation process at STC and in the member First Nations, meet with surveyors, support communities through the surveys.
- Provide expert program advice and guidance on health methods, principles and practices, technical advice and interpretation of legislation to management in member First Nations.
- Develop program initiatives, serve as chair for the Sask. First Nations Home Care Working Group (SKFNHCWG), committee member on the SK FN HCWG subcommittees: Policy Committee, Forms Committee, Palliative Care Committee, Discharge Planning Committee, SHA/FSIN
- Ongoing Quality Improvement practices including:
- review all incident reports and initiate any necessary changes to decrease and or prevent any further occurrences.
- Develop and support Patient Safety, Risk Management and Quality Improvement
- To develop specific Policy and Procedures, chart forms that are consistent with best practice and standards of care
- To develop projects which enhance the quality and delivery of the HCC delivery (practice manual, client records, and informatics and resource materials).
- Promote continuous improvement in care quality and safety.
- 4. Llinkages
- Establish and maintain links and cooperative working relationships with member First Nations, provincial and federal agencies to ensure coordinated approaches for the effective delivery of health care.
- To promote awareness to Provincial Authorities, Regional Health Districts, First Nation Tribal Councils/ Agencies and individual First Nations.
- Establish and maintain cooperative working relationships with outside agencies to effectively deliver equitable and culturally sensitive health care services.
- Link with funding agencies; link/partner with other Health agencies (SHA); collaborate with external partners to develop and initiate new programs.
- Coordinating and participating in the development of Regional HCC plans and peer reviews of plans and needs assessments as requested.
- To work with the National Program Evaluation Study team in collecting appropriate documents, organizing interviews/circles with community members, collection, analyzing and interpreting information. Recommending appropriate changes needed to enhance program delivery.
5. Program Budget
- Maintain stats, complete monthly, quarterly and annual statistical reports, do data analysis and evaluate program outcomes.
- Plan, analyze, advise and forecast financial requirements for the HCC, MCH, KYS, ADI, FASD and CPNP programs, includes preparing a budget, analyzing data to determine allocation of funds to salaries, operations, training, travel, external contracts, specialized equipment, etc.
- Develop and maintain budget spreadsheets and binder to hold all invoices and program expenses for HCC, ADI, CPNP, Nursing Education, MCH, and KYS.
- 6. Community Development
- Create and maintain an atmosphere with the community that builds trust and respect, encourages innovation and promotes meaningful participation, growth and development.
- To provide guidance on community development issues and innovative problem solving.
- To provide consultation to First Nation leadership and communities on service development and provision issues related to recruitment and retention of staff, staff education, professional development, and licensing and liability issues.
- To support the Home & Community Care Program service delivery staff to ensure services are provided ensuring clients attains optimum level of independent functioning.
- To support the HCC nurses with client needs assessments and care plan development as required.
- Advocate for client needs and support health and wellness initiatives within the community.
Working Environment :
- Standard office environment; regular travel (as required)

- Occasional work within high stress situations, while providing leadership and support as a Registered Nurse
- Work is completed in an environment characterized by interactions with people and interruptions.
- Exposure to environment and human factors that increase the risk of contracting disease is moderate.
- Position requires continuous mental concentration and attention to details.
- The work in this position is demanding with frequent pressure of deadlines and demands from a variety of sources.
- Some mental stress is caused form dealing with interpersonal relationships, which require intervention.
- Recruitment and retention of qualified staff in the communities is a constant challenge due to remoteness and wage imparity.
- Tension brought in from within the community effects job productivity.
- Remoteness of communities and lack of funding hinders provision of specific services and necessary medical supplies.
- Flexibility of work hours to accommodate community needs.
- Working in isolation with regards to professional aspects of the program, often no link to communication with other professional bodies when in communities.
- Increasing reporting requirements with lack of technical support, unrealistic deadlines, and lack of funds to improve technology to provide the reporting requirements takes away from providing services.
- Frequent exposure to potential confrontational interaction requires tact, diplomacy and self- control. This position requires personal tolerance and endurance.
Physical Demands:- Freedom of movement; Keyboarding (>2 hrs at a time); ability to take breaks as necessary; Driving and sensory demands
- Sitting for extended periods of time, specifically when travelling to STC member-communities.

Nature and Scope
- Working in conjunction with the STC member First Nations to have the community embrace the Home & Community Care Program and Aboriginal Diabetes Initiative.
- Reporting directly to the STC H&FS Director of Wellness.
- Establish effective working partnerships with First Nations, Nurse Managers, Health Directors, First Nations Health Authorities, First Nations Tribal Councils, Central Bands Working Group, Provincial Working Groups and National Home Care Working Groups, Canadian Diabetes Association, Saskatchewan Health Authority, etc.
- Establish effective linkages with Home Care Association, FNIHB, Health Canada, INAC and other appropriate associations/organizations appropriate to the HCC Program and the ADI Program
- Establish collaborative relationships with Provincial Home Care administrators and service consultants.
- Develop and maintain networks with universities, training facilities, Specialized Home Care practitioners, professional associates and government officials to further the development of the Home & Community Care Program.
- Develop and maintain networks with Diabetes Educators, Physicians, Renal Unit, and Canadian Diabetes Association to further the development of the ADI Program.
Education and Experience:
- Baccalaureate or Diploma in Nursing;
- Experience in Home Care, and in a leadership role;
- Supervisory and program management experience;
- Demonstrated financial literacy that ensures management of department budgets, oversight of financial commitments and contractual obligations and responsive to changing conditions;
- Must demonstrate appropriate communication during difficult, high stress and/or emotional situations. Foresees potential conflict and takes preventative steps. Handles conflict when it arises; assisting with resolution or determining solutions;
- Ability to anticipate changing priorities, identify solutions to improve service efficiencies in program delivery and effectively communicate goals and objectives;
- Current registration with College of Registered Nurses of Saskatchewan or eligible for registration in Saskatchewan;
- Current CPR-BLS, First Aid and AED certification;
- Must possess a valid Saskatchewan driver’s license, a reliable vehicle and meet STC’s insurance requirements; and
- Must provide a current, original, Canadian Criminal Record Check (CPIC) and vulnerable sector check as a condition of employment.

- Knowledge of Indigenous health issues, community dynamics, and traditional healing practices is a strong asset; and
- Ability to speak First Nation language an asset.
