Salary: $19.50 - $24.02 plus $3.00/hr top-up for direct service hours worked
Lambton Elderly Outreach is seeking a compassionate and reliable Personal Support Worker (PSW) who is willing to travel throughout Lambton County to fill a vacant position. The Personal Support Worker is responsible for assisting clients with routine activities of daily living that the individual is unable to perform unassisted. With the direction of the Manager of Home Support Services, the Personal Support Worker performs duties as outlined in the Client Plan of Care and/or Plan for Personal or Respite Care. The duties vary according to the individuals or families receiving service. The Personal Support Worker must work within a goal-directed, outcome based, client-centered philosophy.

Primary Duties:
Personal Care: bathing, grooming, dressing, ambulation assistance, adult care, assist with medical appointments
May assist with specialized care under the direction of a professional
Respite: caregiver relief; client supervision, meaningful companionship, stimulation, and personal care
Home Management: assist with shopping and assist in establishing routines
Other Duties; errands including grocery shopping and picking up essential supplies
Professional and accurate recording after each assignment
Complete client confirmation sheets and have client sign to confirm service
Travel throughout Lambton County to client homes
Other duties as required

To follow the clients Plan for Personal Care and/or Plan for Respite Care, and together with the client, prioritize the routine activities to meet the individuals requirements or goals

To review performance with the program supervisor after probation, and yearly thereafter
To understand and follow the Client Bill of Rights
Requirements:
Grade 12 secondary education or equivalent
PSW, DSW, or HSW certificate from a recognized institution or equivalent level of experience
Valid drivers license, and use of own vehicle
Provide medical certificate stating immunizations are up-to-date
Clear Criminal Reference Check and Vulnerable Sector Screen
First Aid and CPR certification
Required to work, day, evening and weekend hours
Moderate skills with technology is preferred
