The Home Share Coordinator provides guidance, leadership, and ongoing support to Home Share Providers who have welcomed a Supported Individual into their family home. Reporting to the Manager of Home Share, the Home Share Coordinator maintains regular communication with both the Manager and the Home Share Providers, ensuring timely updates and collaboration on all aspects of service delivery.
The Home Share Coordinator is responsible for completing all required administrative duties, including ongoing monitoring visits, reviews, and assessments of the Home Share arrangements they oversee. This role involves meeting regularly with Supported Individuals and Home Share Providers to ensure quality of care, address concerns proactively, and support positive, stable home environments. The Home Share Coordinator also contributes to the ongoing development and improvement of the Home Share program.
As a member of the CLS leadership team, Home Share Coordinators are professionals and leaders accountable for building strong, respectful, and collaborative relationships with Supported Individuals, their families and networks, Home Share Providers, and the CLS Administrative team.
The Home Share Coordinator takes a proactive and solutions-focused approach to problem-solving and relationship-building, while maintaining a high level of thoroughness and attention to detail in all aspects of their work. They role-model CLS values and consistently apply CLS policies and procedures in an ethical, caring, and professional manner, supported by a strong working knowledge of organizational standards and expectations
About the OpportunityIn co-operation with the individual, their networks, their home share provider and team, the Home Share Coordinator will:
- Maintain thoroughness and attention to detail in every aspect of the job.
- Cooperate with the Director of Programs to role-model and actively apply the CLS values, policies, and procedures in an ethical, caring, and professional manner, through sound knowledge of all these aspects.
- Educate contractors and employees regarding the Safety Program Binder and other documents in an ethical, caring, and professional manner, through sound knowledge of all these aspects.
- Attend meetings as scheduled by the Director, and punctually; participate in a positive and respectful manner.
- Arrive on time for scheduled appointments and activities.
- Present a positive and welcoming attitude regardless of personal or professional situations.
- Role-model professionalism at all times, and coach others in a respectful way
- Create and maintain an environment that is emotionally healthy for people to live in by treating all individuals with compassion and respect. The Home Share Coordinator will constantly monitor the quality of this environment.

About YouYou are interested in supporting others and show an understanding of the person-centered and active support approach. Additionally, you have the following requirements:
Required: - Class 5 driver’s license (permanent staff must be willing to obtain a Class 4 license)
- Access to a reliable vehicle
- Experience within a leadership capacity within the field
- First Aid and CPR Certificate
- Clear criminal record (paid by CLS)
- Clean driver’s abstract
- Comfortable using transit systems

Salary Range: 60,000 to 70,000
