Social Worker – Full-Time (Flexible Schedule)
Bluewater Area Family Health Team – Zurich, Ontario
About the Role
- Join a welcoming interdisciplinary team including physicians, nurse practitioners, nursing staff, and another Social Worker
- Work collaboratively to support patients with complex and evolving needs
- Contribute to thoughtful, coordinated, team-based care
What You’ll Do
- Provide counselling and psychosocial support across the lifespan
- Support patients with:
- Mental health concerns
- Addictions
- Chronic illness
- Life transitions
- Balance both brief, solution-focused support and longer-term therapeutic relationships
- Collaborate with team members on care planning and shared decision-making
- Participate in case discussions and team meetings
- Contribute to the development of programs and services that reflect community needs

- Registered with the Ontario College of Social Workers and Social Service Workers
- Strong collaboration and teamwork skills
