Job Summary
The SCOPE (Seamless Care Optimizing the Patient Experience) Mental HealthService Navigator is responsible for providing navigation support for primary care providers related to Mental Health as part of the SCOPE. The SCOPE Mental Health Service Navigator, will be working within the recovery and guided self-help models of care, will support community based Primary Care Practitioners and work closely with consulting psychiatrists at Sunnybrook Health Sciences Centre and Family Navigation Project.
SCOPE Program is a virtual, interdisciplinary program connecting primary care providers (PCPs) in the community to real-time support when they need it most. SCOPE is comprised of 1 Mental Health and Addictions Navigator (LOFT)SCOPE is a partnership program between LOFT Community Service and Sunnybrook Hospital working to support the needs of primary care providers in North Toronto to meet the need of their patients, who are living with mental health and addictions. Through a single point of access, SCOPE quickly connects PCPs to local providers who can help with urgent consultations, diagnostic imaging, curated home and community care, mental health, and navigating other services available in acute care or in the community.
What You Will Do
- Assists clients in the development and implementation of a care plan tailored to the client's needs and issues designed to improve quality of life. Identify and refer to resources specific to those needs and issues;
- Act as a liaison and client advocate among those involved with the client's care including family, other care-givers, medical and other multi-disciplinary providers;
- Provide supportive counselling or referral and linkages to health care, programs or service providers;
- Provide crisis management/intervention and assist in the development of crisis plans for clients;
- Develop respectful and trusting relationships with clients using Recovery and Psychosocial Rehabilitation principles;
- Maintain strict confidentiality and follow all other required policies and procedures;
- Consistently interact with clients, staff, family members, other community agencies and partners in a professional manner that reflects favourably on the organization.
- Attend and actively participate in all required training;
- Liaise, collaborate and coordinate with other professionals, service providers and partners;
- Participates in partnership meetings as required
- Participates in weekly team meetings, individual supervision and regular agency administration meetings
- Takes responsibility for identifying areas for professional growth and training, in conjunction with the Manager or Director
- Participates in ad-hoc committees within the agency, providing input to improve the services provided by the agency
- Represents the Network at external workshops, inter-agency initiatives, staff training and other community-based activities
- Complete other duties, from time to time, as assigned by Program Director/Manager
What You Bring:
Must Haves
- Completion of degree in social services/human services or equivalent
- Minimum 3 years of experience in providing support to vulnerable seniors and older adults with special needs including mental illness, addictions and severe social isolation. Completion of Gerontology is an asset.
- An understanding of harm reduction, and psychosocial rehabilitation principles, the therapeutic relationship, and an ability to come to an agreement with client on goals/priorities
- Crisis intervention skills, with well-developed problem-solving abilities and interpersonal skills
- Group counselling related to mental health, addictions and related issues
- Excellent Management of the privacy and confidentiality of all client information according to agency policy
- Ability to submit time sheets, expense claims and other required information in accordance with agency guidelines
- Good oral and written communication skills and computer literacy, including MS word and Excel
Nice-to-Haves
- Physically able to perform demanding body mechanics (i.e., lifting, transferring, etc.)
- Ability to work effectively in a team environment
- Demonstrated effectiveness working with minimal supervision with proven reliability and trustworthiness
- Ability to communicate in a different language, an asset.
